EPaCCS Patient IDentifiable (PID) Dashboard

What information does it provide?
The EPaCCS PID dashboard shows all individuals within your GP practice who are currently on GSF or Palliative Care registers.
This Information can be managed via the EPaCCS PID Dashboard, where you can view and filter by their GSF stage to identify individuals that may need to be discussed at GSF or MDT meetings.

Who should request access?

Primary Care practice staff (recommended 1-2 users max per practice) who are:

• involved in the care of individuals at End of Life and need regular access to patient data

• actively involved in the management of individuals on palliative care or GSF registers.

How to request access

Please note: If the dashboard is not accessed for 60 consecutive days access will be revoked
