Created with Dementia United and the Alzheimer’s Society, this plan ensures personalised care and support planning is available for everyone involved in a person’s care.

What is the Dementia Wellbeing Plan?
The Dementia Wellbeing Plan provides a single, structured and shareable record of a person’s dementia diagnosis, needs, priorities and support arrangements.
It has been developed in partnership with Dementia United, the Dementia Carers Expert Reference Group, and people living with dementia across Greater Manchester to ensure the content reflects real lived experience, clinical relevance and what families tell us matters most.
Dementia care in Greater Manchester involves multiple services and frequent transitions where inconsistent documentation or siloed information can lead to duplication, delays and avoidable deterioration.
The Dementia Wellbeing Plan standardises how essential information is captured and shared across the system, supporting clinicians to deliver safer, more coordinated and personalised care.

What is the purpose of the Dementia Wellbeing Plan?
- To present a concise, clinically relevant summary of a person’s dementia diagnosis, needs and priorities.
- To provide immediate visibility of key risks (falls, wandering, nutrition, safeguarding), legal status (LPA/DoLS) and carer information.
- To capture personal preferences and communication needs identified through co-production with Dementia United and the carers reference group.
- To support proactive identification of deterioration or escalating need through structured updates and review prompts.
- To strengthen system-wide care coordination, especially during urgent care episodes, crisis response and hospital discharge.
What is the clinical value of the Dementia Wellbeing Plan?
- Co-produced and person-centred: Patient and carer voices shaped the structure, language and priority fields within the plan.
- Reduces duplication: Removes the need to repeatedly gather history or recreate assessments across teams.
- Improves clinical safety: Critical information (allergies, medication adherence, behavioural triggers) is visible at a glance.
- Enables proactive management: Supports early recognition of decline or carer strain to enable timely interventions.
- Optimises integrated working: Provides a single source of truth aligned with neighbourhood MDTs, acute/community pathways and urgent care services.
